Hindsight

Hindsight is 20/20 vision, history is only written by the winners, history has a Christian bias, history has a white European bias and looking back with rose-tinted glasses. Hindsight is a human construct that creates understandable patterns and favourable narratives, telling an apparent, legible storyline. The above biases (teleology, triumphalism, Eurocentrism) interact and influence hindsight bias. It is an old human tendency toward mental narrative inflation – We suffer more in our imagination more often than in reality (Seneca, 4BC-65AD) Fischhoff posits that outcome knowledge inflates certainty, implying that hindsight bias can lead to misplaced confidence, making events feel more foreseeable after the fact (I knew that was going to happen) rather than accurately predicting them (Fischhoff, 1975).
This will have a considerable impact on disaster reporting because it is written after the ending is known and institutions have begun to defend their legitimacy, allocate responsibility, attribute loss, control the public narrative, meaning, and blame, a process known as accountability framing which can intensify Fischhoff’s creeping determinism - the felt inevitability that seeps in once the outcome is known; the narrative hardens, alternatives become hard to imagine and probabilities get rewritten. Reports therefore drift toward neat causal chains, even when the incident was produced by varying processes that did not unfold linearly (Shrivastava et al., 1988, pp. 285–303). Disaster reports have a compounding effect: they are necessary for lessons learnt and for entry into the feedback loop, thus encouraging a linear narrative of events.
Industrial crises are prone to this because warning signals are often buried in routine noise. A build-up of small incidents aligning to cascade to an event. Shrivastava et al (1988) emphasise that crises can be triggered by low-probability events and that warnings are often present yet discounted under normal organisational constraints. These warning signs and their absence are elevated after the event as if they were clear and decisive (Shrivastava, et al., 1988, p. 288). The report then reads like a failure to notice the obvious, when it may have been a rational judgment under usual pressures. Official reports are accountability artefacts, so they reward clarity more than uncertainty. Uncertainty is read as weakness and leaves a knowledge gap that will be filled with a false narrative. Vital N et al (2025) discuss conspiracy beliefs, the information vacuum, the dissemination of misinformation on various websites, how people search for information, and their approaches to analytical thinking. Post-incident ambiguity becomes politically costly, resulting in everyday precursors being framed as warnings, while other views that were plausible at the time are downplayed or omitted. In collective knowledge systems, this same dynamic shows up as a negotiated narrative, as discussed by Oeberst et al (2018, pp. 1010-1026). In their Wikipedia article, they found robust hindsight bias specifically in disaster articles, even when other event categories were less affected, suggesting that the disaster genre invites predictable, sense-making edits.
The Challenger disaster illustrates this problem. The Presidential Commission on the Space Shuttle Challenger Accident (1986) documented that cold temperatures increased the risk of O-ring seal failure and that a contractor had recommended against launches below 53°F. The report found the launch decision flawed because key decision-makers were unaware of the technical concerns. The report continues to state that if the full facts were known, the flight would not have taken place, and that poor communication hindered sound decision-making. The kind of ambiguity that hindsight later converts into certainty, the simplification of the path while forgetting the complication of the journey. NASA created an Office of Safety, Reliability, and Quality Assurance, on the explicit recommendations of the commission, reporting directly to the Administrator, to provide independent oversight of critical flight safety matters following this incident (Donahue & O'Leary, 2012, p. 406).
Hindsight bias can be identified in written accounts by looking for language that signals (“inevitable”, “obvious”, “should have known”), backwards-built chains that highlight the clear path and are less descriptive of credible alternatives available at the time. By comparing personal logs, interpersonal messages, photos, videos, and any other available communication channels alongside the final report, then flag where evidence and confidence do not align. Linguistic marker approaches used to detect hindsight framing in Wikipedia (such as LIWC-style analysis) can be adapted to identify instances in which narrative closure is replaced by decision-context reconstruction (Oeberst, et al., 2018, p. 5; Tausczik & Pennebaker, 2010). Often missed in reports are other pressures: time pressure, limited situational awareness, contested data, and organisational constraints. When these are removed from the narrative, behaviour is judged as if decision-makers had the reporter’s full timeline. That is the core error; hindsight is not foresight (Fischhoff, 1975, pp. 288, 292. 298).
